General personal data of customers or suppliers are processed by Laser&SkinTherapie for the following purpose(s):

  • communication about the assignment;
  • executing or issuing an order.

  • the execution of the agreement.

For the above purpose(s), Laser&SkinTherapie may request the following general personal data from you:

  • infix;
  • surname;
  • address, postal code and place of residence;
  • landline and/or mobile phone number;
  • Date of birth;
  • email address.

Special personal data of customers or suppliers are processed by Laser&SkinTherapie for the following purpose(s):

  • administrative purposes;
  • communication about the assignment;
  • carrying out or issuing an order;
  • being able to tailor the treatment to the individual client, taking into account indications and possible contraindications to treatments;
  • Exclude misapplication of products and/or treatments.

The basis for this personal data is:

  • the execution of the agreement and consent.

For the above purpose(s), Laser&SkinTherapie may request the following special personal data from you:

  • gender;
  • age;
  • profession;
  • medication use;
  • general practitioner;
  • alcohol consumption;
  • Drug abuse;
  • smoking behaviour;
  • sports behavior;
  • Diet;
  • food supplements;
  • water intake per day;
  • photos/videos;
  • sun protection products;
  • sun behavior;
  • pregnancy;
  • allergies;
  • blood pressure;
  • chronic diseases;
  • skin diseases;
  • skin condition;
  • skin care products;
  • acne;
  • couperose;
  • cold sores;
  • rosacea;
  • diabetes;
  • treatment dermatologist;
  • plastic surgeon treatment;
  • hormone therapy;
  • cancer;

Your personal data will be stored by Laser&SkinTherapie for the above-mentioned processing(s) for the period:

  • during the term of the agreement and thereafter for a maximum of 7 years for the financial administration.

3. Provision of personal data to third parties

We may provide the data you provide to us to third parties if this is necessary for the execution of the purposes described above.

For example, we use a third party for:

  • Taking care of the internet environment;
  • Hosting our website;
  • If necessary, referral to general practitioner.

We never pass on personal data to other parties with whom we have not concluded an agreement. Of course, we make the necessary agreements with these parties to guarantee the security of your personal data. Furthermore, we will not provide the information you provide to other parties, unless this is legally required and permitted.

Within the EU

We do not provide personal data to parties located outside the EU.

Minors

We only process personal data of minors if written permission has been given by the parent, guardian or legal representative.

Retention period

Laser&SkinTherapie does not store personal data longer than necessary for the purpose for which it was provided, or is required by law.

Security

We have taken appropriate measures to protect personal data against unlawful processing. For example, we have taken the following measures:

  • all persons who can access your data on behalf of Laser&SkinTherapie are obliged to maintain their confidentiality and have been informed about the importance of the protection of personal data;
  • we have a username and password policy on all our systems;
  • we make back-ups of the personal data in order to be able to restore them in the event of physical or technical incidents;
  • We regularly test and evaluate our measures.

4. Rights regarding your data

You have the right to inspect, rectify or delete the personal data that we receive from you. You can also object to the processing of your personal data (or part thereof) by us. You also have the right to have the data provided by you transferred by us to yourself or on your behalf directly to another party. We may ask you to identify yourself before we can comply with the aforementioned requests.

If we are allowed to process your personal data on the basis of a permission given by you, you always have the right to withdraw this permission.

Complaints

If you have a complaint about the processing of your personal data, we ask you to contact us directly. If we cannot find a solution together with you, we will of course find this very annoying. You always have the right to file a complaint with the Dutch Data Protection Authority. This is the supervisory authority in the field of privacy protection.
